All forms relating to the Gaming Machine Act 1991 can be downloaded from the following categories:

A Payment Authority Form is included with each application/form which requires the payment of a prescribed fee, or can be downloaded here.

To ensure you receive all correspondence sent by QOGR, please notify this Office whenever your contact details change.

For gaming machine sites a Change of Contact Details form must be completed and signed by an Executive Officer e.g. Director/Secretary (not a gaming nominee) and returned to QOGR on fax no. 3872 0864.

Holders of a Gaming Nominee or Gaming Employee licence must complete and sign a Change of Address Notification form and return it to QOGR on fax no. 3872 0957.
